The Importance of Care Instructions

Care instructions and care team details, improve your ability to coordinate care by providing specific guidelines and contact information your PatientPing community.  

Care coordination details, i.e. care instructions and care coordinator contact information, can be assigned based on a patient's attributed Program or Practice, This means that  care instructions and care coordinator assignments can be tailored to specific Programs, such as a high-risk or diabetes group, or to a specific Practice name/location where the patient's provider practices.


Update Care Instructions
Care instructions are submitted by your organization via a template. This template establishes a link between a patient’s affiliated practice or program and map directly to your patient's care team. Care instructions can be assigned to any patient group, which means a patient may have several sets of instructions that apply to them. These instructions can be updated on a monthly basis.

Utilize Care Instructions
Details submitted in the care instructions file allow you to render specific instructions at the patient event level, such as specific guidelines for admitted patients who belong to a high-risk program. This allows admitting facilities to contact their care team at the point of care.
