The patient profile provides valuable information and context to support your care coordination workflows!
Here, you can review your patients' demographic details, care team contact information and instructions, comprehensive clinical visit history, care summary (comprising diagnosis and encounter summaries), as well as any applicable Bamboo Health flags.

*We understand how hard it can be to contact patients whose phone numbers are frequently updated. Therefore, in the demographic section, we collect and show all phone numbers provided for a particular patient across their encounters over time, revealing the most recent number at the top!

Recent Post-Acute Providers |
This section will highlight all post-acute providers through which your patient has received care across the PatientPing network.
Care Summary |
The care summary includes your patient's diagnosis summary and encounter summary. To see more granular information on either of these topics, simply expand your view by selecting the blue caret:
Please note that you can filter the diagnosis summary by the following time periods: Last 30 days, Last 3 months, Last 6 months, Last 12 months, All-time. If there are fewer than 10 results, all diagnosis categories will be shown by default.
Encounter History |
The encounter history is organized chronologically and provides a longitudinal view of your patients’ prior care encounters across the Bamboo Health community. Your patients’ open and ongoing encounters will be highlighted in yellow to the left, and will also be fully expanded. You can expand any prior event by clicking on the blue arrow to the right of the encounter, or you can move your cursor to the top of the encounter history and click on expand all to get more information.
HHA Coverage History |
Your patient’s ongoing and prior home health services will be separated from and shown to the right of the encounter history, under HHA coverage history.
