CCDs through Bamboo Health
In order to provide you with the most actionable data available and keep you informed as you coordinate care, we are now delivering Continuity of Care Documents (CCD) from all Bamboo Health Facilities in the Michigan PatientPing community. This will include the problem list, allergies list, and discharge medications resulting from a patient’s hospital encounter.

| These documents can be accessed in the patient profile under the attachments section. All CCDs received for a specific patient are ordered by the date of the encounter. These documents will be labeled by the facility from which they were generated in addition to the date of that hospital encounter. Download CCD Files |
| Select any of the CCD links to download the file. Clicking on this link cause a pop-up window to appear, providing you with a snapshot of the CCD file, including:
From this pop-up window, you will have the option to either download the full CCD as an XML file or print the CCD summary as a pdf. The printed summary will contain the exact same data viewed from the pop-up window. In order to maintain data integrity, the XML file is exactly the same as the version we receive from the sending hospital and will require XML viewing software to open. If you need a suggestion for XML viewing software, we suggest you ask your IT team. |
